Required documents
To Credential with Health Net California, what documents are required?
Fill out the online credentialing form. Select Credentialing, then your state, and then Create an Account on ActivHealthCare.com – if you already have an account, see Re-Credentialing.
- W-9 – This form can be found on the AHC website under Credentialing or Forms. Please fill out the W-9 form with the information provided by the IRS and the Tax ID number you will be using to file your claims. This could be your company name and TIN, or your personal name and Social Security number.
- Release Authorization – this document certifies that all of the information you provided on the online application is true and correct.
- ActivHealthCare Provider Agreement – this is ActivHealthCare’s contract.
- Network Option Form – Use this form to decide whether you want to enter into an AHC contract with each of the networks listed. For each network, choose whether to opt in or out.
- Ambetter Option Form – To join Ambetter, you must fill out this form.
- EFT (Electronic Funds Transfer) – You’ll need this document to deposit checks from AHC directly into your back account.
- Certificate of Malpractice Insurance – One page containing your name, coverage amounts (minimum coverage is $1,000,000/$3,000,000), and expiration date. By adding AHC as a certificate holder, your office will avoid having to update this every year.
